HIP PROCEDURES

 

Arthroscopic Surgery

Hip arthroscopy is a relatively new surgical technique that can be effectively employed to treat a variety of hip conditions. Arthroscopy is a surgical procedure in which an arthroscope is inserted into a joint. The benefits of arthroscopy involve smaller incisions, faster healing, a more rapid recovery, and less scarring. Arthroscopic surgical procedures are often performed on an outpatient basis and the patient is able to return home on the same day.

Labral Tear Repair – Femoroacetabular Impingement (FAI)

Femoroacetabular Impingement (FAI) is a condition resulting from abnormal pressure and friction between the ball and socket of the hip joint resulting in pain and progressive hip dysfunction. This when left untreated leads to the development of secondary osteoarthritis of the hip.

Bursitis Treatment

A bursa is a small, soft sac located throughout the body, particularly at bony prominences, and their purpose is to cushion and reduce friction. Bursitis is inflammation of the bursa, either by low grade repetitive trauma or one major injury like a fall or contusion. The most commonly inflamed bursa in the hip is the greater trochanteric bursa, which is on the outside of the hip. Symptoms include pain when walking, pain on the hip side with direct pressure, and sometimes snapping. This is typically treated with therapy, medications, and/or injections. However, if the pain does not improve with nonoperative measures, surgery may be necessary.

The bursa can be removed arthroscopically and is an outpatient procedure. In some cases, a group of muscles called the abductor muscles, which lie underneath the bursa, may be torn in addition to the inflammation. If this is the case, these may be repaired at the same time as the bursectomy. This can be done arthroscopically or open, depending on the size of the muscle tear.


Avascular Necrosis (AVN) Treatment

Avascular necrosis (AVN), also called osteonecrosis, is a condition where part of the bone loses blood supply and the bone gradually dies. This can be caused from steroid use, chemotherapy or radiation treatments, or have no known cause at all. It is a progressive disorder than can eventually lead to collapse of the bone that is affected. If caught early, however, sometimes surgery and/or medication can be used to stall or reverse the progression.

Dr. Venu Madhav specializes in a procedure called a core decompression, where the area of necrosis (dead bone) is drilled out. The good bone that surrounds the necrotic bone brings blood into the area and encourages healing. This is also used in conjunction with bone graft mixed with a patient’s own bone marrow cells being placed in the area where the necrosis is drilled out. The goal is to revascularize and promote bone healing. This is only possible in stages of AVN where there is no collapse, but can be an effective joint preservation procedure.

PRE-OP COURSE

We recommend stopping use of all anti-inflammatory, fish oil supplements, and other blood thinning medications one week prior to surgery. Your clearing doctor will make recommendations regarding other prescription medications.


POST-OP COURSE

Your hospital course will entail admission from 1-2 days. Once you are cleared by therapy you can be discharged to home with provided visiting nurse services and physical therapy services at home. This will be followed by outpatient Physical Therapy. Your initial follow-up will be six weeks postoperative.On average your return to work recovery will be approximately six to eight weeks, depending on individual recovery. The majority of patients will be able to resume most sporting activities in twelve weeks post op.

 Anterior Cruciate Ligament (ACL) Reconstruction

The anterior cruciate ligament is one of the major stabilizing ligaments in the knee. It is a strong rope like structure located in the centre of the knee running from the femur to the tibia. When this ligament tears (acl tears) unfortunately it doesn’t heal and often leads to the feeling of instability in the knee.

Sometimes the ACL is injured with other ligaments, and we will treat those ligaments:

  • Posterior Cruciate Ligament (PCL)

ACL reconstruction( acl surgery) is a commonly performed surgical procedure and with recent advances in arthroscopic surgery can now be performed with minimal incisions and low complication rates.

Meniscus Surgery

Meniscus tear is the most common knee injury, and can be either acute or chronic. The meniscus acts as a shock absorber for the knee, and if it is torn, it can cause pain, stiffness, swelling, catching, and/or locking. Sometimes if the tear is small, nonsurgical treatment such as injections and physical therapy may be sufficient. If surgery is required, it is usually done arthroscopically, which is a minimally invasive technique that uses a small camera and instruments inserted through 2-3 small incisions to either repair the damaged meniscus or remove the torn portion if it is determined to be irreparable. Care is always taken to preserve as much meniscus as possible.

Patella Instability Treatment

Some people can have instability of the kneecap (where the knee cap pops in and out of place) which leads to chronic pain and decreased function. This can be due to a trauma that dislocated the patella versus chronic instability that did not have a trauma. Usually the latter is due to an abnormality in the patellofemoral structure. Instability can be treated with physical therapy focusing on strengthening and stability braces, but sometimes surgery is required. Surgery can be as small as a minor arthroscopic procedure to clean out loose bodies, repair or reconstruction of the ligaments that hold the patella in place, or more extensive realignment procedures.


Knee Preservation

Osteoarthritis is typically caused by degenerative wear and tear over time. Unfortunately, sometimes a severe injury or deformity can cause arthritis to develop early or in isolated areas of the knee. In order to avoid doing a total knee replacement, an osteotomy may be an option to preserve a patient’s own joint by realigning the knee. This can transfer pressure from the arthritic part of the knee to a healthy part of the knee to decrease pain and increase function, as well as put off a knee replacement for several years. Another option may be a partial knee replacement, which is a resurfacing of only the area that has arthritis. There is minimal bone taken when compared to a total knee replacement, and the remaining portion of the knee is preserved.

Cartilage Defects

Cartilage is smooth, white tissue at the end of two bones that come together to make a joint. It provides a smooth surface so the joint can glide. Cartilage can wear and tear slowly over time, which is called osteoarthritis. Sometimes, however, an isolated area of the cartilage can become damaged, usually in a trauma. Since cartilage does not heal on its own, it needs to be replaced. There are several different techniques: microfracture, autologous chondrocyte implantation, osteochondral autograft or allograft transplantation. Most procedures are outpatient procedures. A clearance may or may not be needed based on your medical history. If one is required, this may be done by you PCP, and we will provide you with what tests are necessary. This must be done within 28 days prior to your surgery date.

Post-Op Course

Most knee procedures are outpatient and you will be going home at the end of the day. You will receive crutch education prior to your release. For most procedures we recommend beginning Physical Therapy within three to five days. You will be given a packet containing PT prescriptions and home instructions at discharge time.

Typical office follow up is ten to fourteen days after surgery, except for meniscus surgery which is six weeks. Generally meniscus surgery require minimal crutch time and recovery time, reconstruction surgeries require an average of four to six weeks use of crutches. Most patients will begin a light Jogging program three to four months after surgery and resume full sport activities at six months to eight months. For meniscus surgeries patients typically resume full sport activities in six to eight weeks.

Shoulder Procedures

 Rotator Cuff Repair

The rotator cuff is collection of 4 muscles that help lift and rotate the shoulder. Most tears occur over time due to degeneration, but some are acute. You can also have a partial or complete tear, and tear more than one tendon at a time. The most commonly torn tendon is the supraspinatus. Common symptoms of a rotator cuff tear include pain with lifting of the arm, weakness, and pain at night. Physical therapy can be helpful to strengthen other muscles and improve function and pain, but if a tendon has a complete tear, surgery is usually the best option because the tendon will not repair on its own. Surgery is generally arthroscopic, and can usually be performed as an outpatient. Steroid injections are rarely indicated in rotator cuff tears because it can make the tear worse by weakening the tendon.

Shoulder Instability Treatment

The shoulder is the most mobile joint in our body, which means it is also at the most risk for instability. Typically, instability of the shoulder is caused from a traumatic dislocation. This can often be treated with rest and physical therapy, but if the shoulder continues to dislocate afterwards with minimal force, then the instability often needs to be fixed with surgery. The capsule and labrum are repaired and tightened to better stabilize the shoulder. This is typically done arthroscopically and on an outpatient basis.

Labrum Repair

The labrum is a soft tissue ring which acts as a seal and stabilizer of the shoulder joint. This can become damaged either in an acute injury or trauma (common in overhead athletes), or degenerate over time. Symptoms can include pain particularly with rotational movements, catching, locking, and decreased range of motion. Sometimes with small labral tears, physical therapy, medication, and/or an injection can be helpful. If surgery is required, the labrum can be repaired arthroscopically. In certain cases, the bicep tendon (which attaches to the labrum) will need to be moved off the labrum and to a different location in the shoulder, and this requires a small incision. The surgery is an outpatient procedure.


Impingement Treatment

Sometimes the rotator cuff is not torn, but it can still cause pain. This is often referred to as cuff tendonitis, bursitis, or impingement. This is because part of the rotator cuff lies underneath two bones called the acromion and clavicle. There is a bursa (a sac that provides cushion) between the bones and rotator cuff to protect it, but with repeated overhead motion, the bursa or the rotator cuff can become irritated. This is because these two structures can become “pinched” by the bone when the arm is lifted. In most cases, this can be treated non-operatively with physical therapy, medications, and/or injections. Sometimes surgery is necessary to remove part of the bone to relieve pressure on the underlying structures. This is an arthroscopic and outpatient procedure.

For surgeries that do not involve a repair a sling is worn for only three days, however, for surgeries that do require a repair a sling is worn for four to six weeks. For surgeries that do not require repair, typically patients may resume full sport activities in six to eight weeks, for surgeries that do require a repair, patients typical resume full sport activities in four to six months.
